You receive a written report with functional documentation you can read against your own case file.

What you receive

• A description of how the consumer’s disability limits the specific tasks of the educational or vocational goal, each tied to the record it comes from.
• The tools we tried with the consumer, what happened, and how each recommended tool addresses each limitation.
• The records we reviewed, listed by title and date, and a plain statement of what the records do not establish.
• A proposed training plan written around the consumer’s own goal and learning style.

Questions about funding, eligibility or categories are yours to decide. The report is written to give you the functional documentation for that decision.

How to refer

Send referrals in writing. This website has no forms and collects no information, so nothing should be entered here.

1. Write to the address on the contact page.
2. Say which subject area applies, and attach or point to the referral materials your office normally sends.
3. Send consumer details by encrypted email.
